The other day, I received information about the exam from the Japan Sports Association.

If you wish to take the exam this year, please check the following information and download the application form.
Please apply through our school.
There is only a short time left until the deadline, so we need your prompt response, but we appreciate your cooperation.

If you wish to take the exam, please see below 4 Please prepare the points.
① Theoretical exam application form (with photo attached) *This year, please attach a copy of your BLS certification on the back.
BLS Regarding qualifications, please refer to the above "First aid (BLS) Regarding qualifications.pdf Please check ".

② Two photographs for the examination ticket (4cm wide x 3cm high: taken from the front with no hat within six months prior to application)
* 1 Please attach this to your application form. 1 Please keep this sheet with you and attach it to your examination ticket when you receive it.) You will also need to bring it with you when taking the practical exam. 2 pieces will be used.

③ Copies of the completion certificate (general and specialized) - 2 copies each (consolidate onto one A4 size sheet)

④Exam fee: 16,500 yen

Once you have prepared all the necessary documents, please submit them to us. After submitting the documents, we will send you a transfer slip for the exam fee by mail.

・Copy of the certificate of completion (general/specialized) 1 Total of each 2 Pieces (A4 size 1 (Consolidate and store on one sheet)
・Address・Telephone number・ mail A note with your address written on it *For sending the transfer slip
